Social Prescribing, a new route to wellbeing

We're leading the way to better wellbeing through providing practical and accessible Social Prescribing at all GP Surgeries across Wrexham.


Did you know?

A fifth of people who go to see their doctor have social problems which result in poor health, anxiety, low mood, grief, loneliness or financial worries.

GP's and other healthcare professionals can make direct referrals for patients to see the Social Prescriber at their local surgery, or alternatively encourage patients to make contact themselves via the Surgeries Reception team.

Funded through Betsi Cadwaldr Health Board and GP Clusters



A social prescriber can help you establish what matters to you and support you to access support and services to reach your health and wellbeing goals.
